You must first be a registered patient with us. You can register in 5 minutes on your first visit. Or, if you prefer, you can fill out and submit the online Patient Registration Form provided in the Registration section of this website.

We accept Cash, Personal Checks, or Debit card with Chip.

We are just getting credit card processing again, for cards with PIN numbers only. It works like an ATM so there is a small charge, but we shall cover that for larger orders.
